| Purpose |
| Under Ohio Revised Code (O.R.C.) 5119.391, the Department of Behavioral Health (DBH) is required to monitor the establishment of recovery housing residences in Ohio. The statute defines a recovery housing residence as “a residence for individuals recovering from alcohol use disorder or drug addiction that provides an alcohol-free and drug-free living environment, peer support, assistance with obtaining alcohol and drug addiction services, and other recovery assistance for alcohol use disorder and drug addiction.”

| To assist DBH with this monitoring function, O.R.C. 5119.391 requires each person or government entity that will operate a recovery housing residence on or after October 3, 2023, including any recovery housing residence that was established and in operation prior to that date, to file with DBH the following form. |
| *PLEASE NOTE BOTH OF THE FOLLOWING: |
- If a person or government entity operates more than one recovery housing residence, a separate form must be filed for each address. If in doubt regarding whether a separate form must be completed in a particular situation, please contact DBH at RecoveryHousing@dbh.ohio.gov
- A form does not need to be filed for a recovery housing residence that is NARR level 4 (see National Alliance for Recovery Residences (NARR) Levels of Recovery Housing Residences, below). In the State of Ohio, NARR level 4 recovery housing residences are considered residential treatment and must be certified as such by DBH under Ohio Administrative Code (O.A.C.) 5122-29-09. | Filing Deadline |
| The due date by which a recovery housing operator must file its first form with DBH depends on whether the recovery housing residence began operating before October 3, 2023, or on or after that date: |

| Residence began operating before October 3, 2023 | November 3, 2023 |
| Residence began operating on or after October 3, 2023 | Not later than 30 days after the first resident occupied the residence. |

| A recovery housing residence is considered to have begun operating on the date that the first resident occupies the residence. |
| Updating Forms |
| If there is any change to information a recovery housing residence operator has filed with DBH, the operator must file a new form and answer “yes” on that form to the question asking whether the form is an update to a previously submitted form. Depending on the circumstances, it is possible that a recovery housing residence operator will need to file updated forms on a continuing basis. |

| The National Alliance for Recovery Residences (NARR) has established a national standard for recovery residences.
